Player/Team Forms:
***Information about KSYSA's NEW Drop/Transfer Form***
Parents, Team Managers and Coaches:
We are now using a NEW Drop/Transfer Form. This form is taking the place of the “Supplemental Roster” and the “Request for Release/Transfer”. We hope this form is easier and more efficient for all involved.
To have a player dropped from his/her team, you must have Part A of the form completed. The League Registrar of your current team will drop your player and you will keep this form for your records.
To have your player transferred to another team, Part A must be completed and then you will use either Part B(transfer in the same club) OR Part C(transfer in a different club) to get your player transferred. The new League Registrar must see Part A is completed to confirm a transfer may take place.
If you are using Part C, you must allow 48 hours for the transfer to be completed by the State Registrar in the state office.
This form must be completely filled out.
League Registrars:
When a Parent, Team Manager or Coach presents you with a Drop/Transfer Form, you simply need to drop the player in League One. The person who gave you the form will keep the form. Please make a copy if you need one for your own records.
When completing Part B (Transfers in the same club), you may use this form however necessary. Because these are interclub transfers, if you have your own procedures or forms, please use them.
When completing Part C (Transfers to a different club), 48 business hours are required for completion by the State Registrar. Once the transfer has been completed, the State Registrar will contact you so you can print the new player card and roster.
The state office DOES NOT need a copy of this form if a player has been dropped or transferred to a team in the same club. If a player is transferred to a team in a different club, the Drop/Transfer Form will be faxed or emailed to the state office.
NOTE:
If you are a Kansas resident playing on a team from another state, you will need to send to the Kansas Youth Soccer office:
1) Kansas Membership & Medical Release Form,
2) Kansas Interstate Permission Form,
3) Proof of age document,
4) Name of team, club and league, and
5) Registration fee.
If you are a Kansas resident needing to register as an At Large Player (not associated with any team),
you will need to send to the Kansas Youth Soccer office:
1) Kansas Membership & Medical Release Form,
2) Proof of age document, and
3) Registration fee.
